All courses that involve diving require:
Medical Form
Every student must complete a PADI Medical Statement and submit all 3 pages of the form to your Instructor before class begins.
If you answer “YES” to any questions requiring medical clearance, or if you are enrolled in a professional or technical diving course, you will need a Doctor’s signature on Page 3 before we can take you diving. PADI requires that the medical form is signed by an MD, DO, PA, NP or Navy Corpsman. If anyone from that list other than an MD signs it, they need to include the facility stamp.
